Fabian Schwendinger, PhD, is a researcher at the intersection of physical activity epidemiology, wearables, and cardiovascular health. His work focuses on developing and applying data-driven methods to understand how daily movement patterns, captured through wearable sensors, relate to longevity and disease risk in diverse populations. His broader goal is to enable more personalised, evidence-based recommendations for physical activity to improve population and cardiovascular health across the lifespan.
Activity intensity strongly associates with longevity
Wearable-derived analyses show a steeper dose-response for activity intensity than for volume regarding cardiovascular and all-cause mortality. Replacing slower with brisker movement is associated with additional benefit, while the largest gains occur when moving from inactivity to moderate activity.
Age and sex modify activity effects
Younger and middle-aged adults show stronger intensity-related associations with lower cardiovascular mortality than older adults. Women appear to benefit more from higher-intensity activity, whereas men and older adults appear to benefit relatively more from higher overall activity volume, including lower-intensity activity.
Adiposity influences cardiovascular benefit patterns
Adults with more favorable waist-to-height ratio show stronger associations of both intensity and volume with lower cardiovascular mortality. In higher central adiposity, associations appear attenuated, and greater total activity volume achieved through longer duration appears relatively more relevant than higher intensity alone.
In the seminar “How do age, adiposity, and sex modify the effect of physical activity for cardiovascular longevity?”, organized by Klinik Barmelweid, PhD Fabian Schwendinger reviews epidemiological and accelerometer-based evidence on the association between physical activity and cardiovascular health. He outlines the historical basis of physical activity epidemiology, from the London busmen study to current WHO recommendations, and emphasizes that objective wearable-derived measures may characterize dose–response relationships more precisely than self-reported questionnaires. The presentation indicates that daily step count is a practical metric, with the greatest health gains occurring when individuals move from inactivity to moderate activity, and with approximately 7,000 to 8,000 steps per day representing a desirable target. Dr. Schwendinger further explains that physical activity intensity appears to be a stronger driver of reduced all-cause and cardiovascular disease mortality than volume alone, based on device-derived metrics such as the intensity gradient and average acceleration. Using NHANES data with approximately 8,600 adults and an 81-month follow-up, he examines interaction effects showing that younger and middle-aged adults may derive greater benefit from higher-intensity activity, whereas in older adults greater total activity volume and duration may be relatively more relevant. He also reports that women may benefit more from higher-intensity physical activity, while men show a pattern suggesting benefit from a combination of intensity and duration, and that favorable body morphology is associated with stronger protective associations than higher central adiposity. The session concludes that any increase in physical activity is beneficial, that adherence over years is essential, and that activity recommendations should be individualized, particularly in people with disabling conditions or high fatigue.
